Are you an Advanced Nurse Practitioner /Advanced Clinical Practitioner looking to join a team of forward thinking Urgent Care practitioners delivering high quality patient centered care?
Working across the Urgent Care Service with an allocated base, you will provide safe, high quality primary care services appropriate to the needs of the practice populations on behalf of Lincolnshire Community Health Services
The service is actively involved in the prevention of inappropriate admissions into secondary care undertaking health assessments in the Out of Hours period. Continuous liaison with other agencies/stakeholders ensures continuity of care for the population of Lincolnshire throughout the entire day.
As an autonomous practitioner working within a team, you will be involved in providing face-to-face contacts at our Centres as well as home visiting and telephone consultations. You will use the SystmOne clinical system to input consultations.
Flexible working patterns are essential as the service covers 24 hours a day, to include weekends, evenings, nights and bank holidays and on occasions shifts may need to be covered at short notice.
You will have first level adult registration with the NMC or HCPC registration along with BSc (Hons) Nurse Practitioner/Autonomous Practitioner Degree or equivalent experience and Masters Degree or equivalent experience.You will also hold ENB 998 or equivalent.
This post requires that you have the ability to travel between locations to carry out your work and to attend training/meetings.
