The post holder will need to have a clear understanding of the challenges faced by primary and secondary care in meeting the recommendations for the development of diabetes services, such as those contained in the National Services Framework for Diabetes.
To undertake a holistic, unified assessment process consistent with the approach to Single Assessment in partnership with all agencies, to ensure that acute admission is prevented where possible and that this approach identifies and delivers services to meet and sustain patient’s needs.
To provide expert clinical advice commensurate with professional background. To lead by example, promoting staff motivation, thereby ensuring that high standards of professional behaviour and appearance are maintained at all times.
The main responsibilities of the post:
To undertake defined project work to contribute to the development of the Specialist role across multiagency teams, and work with others to achieve service aims and objectives.
Contribute to the development of the approach to the management of long-term conditions within the trust, by working with the senior clinical team to improve quality of services, through use of evidence based practice projects, standards, audits and measuring outcomes. Making recommendations for change where required.
To provide spontaneous and planned advice, teaching and instruction to patients and/or relatives, carers and other health and social care professionals to promote an understanding of aims of any given intervention and to ensure a consistent approach to patient care.
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
