Sussex Community NHS Foundation Trust is opening a new ward at Crawley Hospital and there are exciting opportunities for Advanced Nurse Practitioners to join the team.

This post offers you as an ANP the opportunity to use your clinical skills and knowledge in the community hospital setting, help shape the model of care for our patients and be part of the development of this new service.

Utilising your skills and competencies you will assess, diagnose and prescribe for patients with complex and/or urgent health needs, manage the episode of care, and liaise with acute and community colleagues as appropriate.

You will work within the multidisciplinary team to help prevent acute admissions and facilitate timely discharges.

You will provide mentorship, teaching and support across the multidisciplinary team, and peer support to ANP colleagues.
