Central Manchester University Hospitals’ new model for delivering integrated care close to patient’s homes is emerging and requires forward thinking individuals with exceptional interpersonal and communication skills to be part of the team leading and developing this initiative. A unique opportunity has arisen for an experienced and motivated individual to join a new and innovative Community Integrated Team.
As an Advanced Nurse Practitioner you will utilise your advanced clinical expertise, levels of judgement, discretion and decision making to deliver excellent clinical care demonstrated through the Trust’s Advanced Competency Framework. You will work closely with the Integrated Team Manager to ensure that care is planned, developed and implemented to meet the needs of the locality. You will proactively case manage and anticipate the future care needs of people at risk of deterioration and will provide a response to urgent health needs in the locality supported by the MDT and specialist services. Incorporated into this is the exciting opportunity to be a key player in the development of Urgent Care First Response for the locality, working closely with other health and social care providers to promote place based care. This role will enable you to lead improvements into standards of care through, supervision of practice, clinical audit, evidence based practice, teaching and supporting professional colleagues and also by contributing to the quality and clinical effectiveness agenda through research and audit.

You’ll enjoy the chance to work with the local community and colleagues across the district and you will be encouraged to help develop and improve the service. You will be a 1st level registered nurse with in-depth professional knowledge, evidenced by appropriate training and a reflective portfolio of evidence. It is essential that you have significant post registration experience, with comprehensive and relevant experience at B7 level educated to Advanced Practice Masters level. Forward thinking, committed and proactive, you will be keen to develop the role and enhance the service and you should be capable of coaching and guiding staff. A natural leader, you will have the ability to manage performance and handle challenging situations with calm and focus. Good communication and interpersonal skills are essential and you will be someone who gains respect and credibility from others.
The Division of Medicine and Community have a vision to deliver integrated care across health and social care boundaries within Manchester. To achieve this vision we intend to educate and develop a flexible, multi-skilled nursing workforce able to challenge and change practice, reduce service barriers and provide care where it is needed within an established team. A full process for developing and maintaining competencies is in place with support from a team of advanced practitioners and medical colleagues. You will receive a comprehensive induction, regular 1:1's and annual appraisals to develop knowledge and skills where appropriate.
