Our Health Reablement Teams are seeking enthusiastic and dedicated Assistant Practitioners with a sound knowledge and experience of working with complex, frail and older people.
Citycare’s Health Reablement services deal with a wide range of cases, providing care and rehabilitation for people presenting with a plethora of multi pathologies, as well as challenging medical and social circumstances.
We are seeking Assistant Practitioners with a wide range of holistic health and social care skills and experiences to complement our service. Visiting patients within their own homes, the teams are dedicated to preventing unnecessary hospital admissions and promoting independence and quality of life enhancement. You will be working closely with Occupational therapists, Physiotherapists and Nurses as well as GPs, District Nurses, Community Matrons, the acute trust and a range of Health and Social Care Professionals.
The aim of our service is to prevent unnecessary hospital admissions and to facilitate timely and safe discharges from the acute setting. We promote independence and with intensive rehabilitation and assessment, we strive to help our service users to achieve their goals, however big or small.
Providing holistic assessments and a range of skills within the AP competency framework, the Assistant Practitioner will be expected to complete initial assessments and follow ups within their competency framework and under the guidance and supervision of the Clinical staff.
The hours of work will be 7am- 11pm on a Rota basis. The Service operates 7 days per week, 365 days per year
