Dundee Health & Social Care Partnership Dundee Keep Well Community Team Assistant Practitioner – Band 4 Based in Kings Cross, Dundee Fixed Term/Secondment until 31 March 2018 [Salary Scale: £19,902 - £22,683 per annum pro rata] A vacancy has arisen within the Dundee Keep Well Community Team for a forward thinking individual to join the team in the post of Associate Practitioner. Keep Well has a broad aim of reducing and managing cardiovascular disease and other associated conditions with a focus on Health Inequalities and developing anticipatory care approaches in health. The successful applicant will support the work of the Keep Well Team and other locality based Health Inequalities teams to develop and enhance approaches to improving health for those who live in geographical areas associated with ill health, and with a range of vulnerable populations including those who are homeless, are affected by substance misuse, offenders and carers. A flexible approach to work is required due to the evolving nature of the work and to meet the often complex needs of the client group. Some evening and week-end working may be required. You should be qualified to SVQ level 3 or equivalent or have proven experience of working at this level. You will be required to achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and to comply with the Code of Conduct throughout your employment.
