Job Description

Hampshire Hospitals NHS Foundation Trust provides services through its three hospitals in Andover, Basingstoke and Winchester. We provide all general hospital services for planned and emergency care as well as specialist services. These include Pseudomyxoma Peritonei for which we are one of the only two national centres treating this rare abdominal cancer. The Trust also provides services at the Countess of Brecknock Hospice in Andover and outpatient services are provided in Alton, Bordon and Romsey.

The Dementia Team at Hampshire Hospital Foundation Trust are looking to appoint an Associate Practitioner to work within their team. The Associate Practitioner is an essential valued member of the team appointed to support the Dementia Specialist Nurses in the delivery of patient care. They provide clinical support and undertake activities such as assessing patients for confusion, supporting patients/families and carers, providing training to staff and group activities for patients.

They will also be responsible for supporting the delivery of data quality and support the team with the collection/storage/retrieval of data and forwarding the information for CQUINN reports, projects and regular service reports.

The successful candidate will be required to work under direct and indirect supervision of the Dementia Specialist Nurses.
