Band 5 Community Nurse – Peterborough City, Whittlesey and Oundle

£21,909- £28,462 pa. For part time posts, the salary is pro-rata

A great opportunity has arisen, for experienced, passionate and enthusiastic individuals to shape and form our new innovative and crucial service.

Cambridgeshire & Peterborough NHS Foundation Trust (CPFT) is implementing an exciting brand new model of care built around integrated neighbourhood teams of community, mental health, acute and social care staff. CPFT’s new Joint Emergency Team (JET) will also play a vital role in this model to help improve services for the people we serve.

We are looking for people who:

  • Have 1st level registration Adult Branch (Active on the NMC register)
  • Have a teaching and assessing qualification or the equivalent experience
  • Have experience of working positively within a multi disciplinary team
  • Have experience of working with older people in a variety of settings
  • Have excellent communication and interpersonal skills
  • Are clinically expert

You will be responsible for:

  • Supporting the Team Leader in enabling the safe management of the Integrated Care team
  • Identifying patients who would benefit from care within, or closer to their home
  • Promoting faster recovery from illness, maximising independent living and preventing unnecessary admission to hospital
  • Playing a clinical role within a multi disciplinary team, providing skilled nursing assessments, treatment, rehabilitation and advice to patients and carers
  • Undertaking assessments to determine patient need and collaborating with key partners to achieve the best outcomes for the patient
