An opportunity has arisen for a highly skilled, enthusiastic and motivated individual to the Reablement service across Nottingham City.

The service consists of multi-disciplinary teams that include Physiotherapists, Occupational Therapists, Nurses, Registered Medical Assistants, Assistant Practitioners and Rehabilitation Support Workers. The aim of the MDT is to promote independence through rehabilitation and crisis management with citizens in their own home or within a short term care setting to prevent unnecessary hospital admissions and support timely hospital discharges.

The role includes delivery of efficient and effective services in line with local, national and professional agendas.

An understanding of Health and Social care provision and ability to work closely with partner organisations and other stakeholders is essential. You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation and Reablement programmes, as well as complex discharge planning experience.

You will need to demonstrate high standards of evidence based clinical practice. You must have excellent interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.

We are looking for enthusiastic, motivated, conscientious & experienced Nurse to work with the established Reablement Teams. We are a dynamic, developing organisation with well recognised rehabilitation services providing education, advice, support and rehabilitation for patients across health, social care, care homes and the independent sector.

You will need to demonstrate how you meet our values and behaviors within the job description & person specification. A ‘can do’ attitude and positive nature are essential. You must be able to prioritise tasks and manage time effectively, also be able to work flexibly within the team and be confident in completing individual tasks.
