Job Description

Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) is a significant provider of high quality mental health services across a core catchment area covering Bath and North East Somerset (B&NES), Bristol, North Somerset, South Gloucestershire, Swindon and Wiltshire. The Trust also provides specialist services for a wider catchment extending throughout the south west.

Our aim is to enable and empower people to reach their potential and to live fulfilling lives through providing recovery and reablement focused services that yield positive outcomes for our service users and their carers.

Specifically, The Trust provides services for people with mental health needs, with needs relating to drug or alcohol dependency and mental health services for people with learning disabilities. We also provide secure mental health services and work with the criminal justice system.

Increasingly AWP provides treatment and care in people's own homes and other community settings, reflecting the preferences of our service users. Our community services are supported by high quality inpatient services that provide short term assessment, treatment and care.

This is an exciting opportunity to join the Mental Health Primary Care Liaison and Memory Service (PCLMS) in North East Wiltshire. This service provides a critical function in delivering safe and effective mental health services within the local area.

As the primary entry point for all new referrals to the specialist mental health teams; PCLMS works with GP’s and other primary care colleagues to effectively triage and assess needs of individuals and facilitate appropriate care pathways.

This is an ageless service and works across the full spectrum of mental health conditions. PCLMS also works with GPs and other professional groups to provide expert advice on the management of mental health problems with the primary care setting.

You will lead a team of highly skilled mental health clinicians and provide operational line management to ensure excellence in service delivery. You will work collaboratively with GP’s, primary care clinicians and AWP’s specialist mental health services to guarantee effective care pathways.

Significant experience and knowledge in community mental health practice is essential for this role

This role is currently based in Devizes but will have the scope to work flexibly in North East Wilts including bases in Chippenham
