Bank Telehealth Nurse, Band 5

We have an exciting opportunitys for highly motivated and enthusiastic Nurses to join our Telehealth team on bank hours. The Telehealth central clinical service supports people living with long term conditions to manage their health using Telehealth technology within their home. The Telehealth Bank nurse, under the direction of the band 6 nurses, will be part of a team who respond to alerts and offer advice and education to patients by phone with the aim of helping them to reduce the number of visits to their GP and unplanned admissions to hospital.

The Nurse will have excellent clinical assessment skills and will liaise with General practitioners and other health professionals regarding patients’ progress. The post holder will have experience in caring for people living with long term conditions such as Heart Failure and Chronic Obstructive Pulmonary Disease and have excellent communication and IT skills.
