Job Description

Salaried GP for New Innovative Service – Proactive Care

As part of the national Proactive Care programme, Brighton and Hove are transforming the way we care for people at risk of losing independence. We wish to shift the focus from 'What's the matter with you?' to 'What matters to you?” The city has formed 6 clusters of GP surgeries and is aligning community services around these clusters. We are using risk stratification software to highlight at risk groups, undertaking whole person assessments to understand their goals and then utilising a team of community practitioners including the third sector, to achieve them.

The ultimate aim is to recruit a citywide team of GPs to support the delivery of care for people within these Proactive Care clusters. GPs have already been successfully recruited for 3 clusters, but we are now looking to support the employment of further GPs.

This new role offers an exciting opportunity to be part of a true community team, delivering care that matters to people. This could mean that you deliver ProActive Care directly yourself, or by supporting practices where a need is identified by providing direct patient care in GP surgeries or home visits, enabling existing GPs to deliver the extended appointments planned.

Working at the heart of a community team, you will be providing Cluster wide support in a flexible way, providing care where there is most need. We are willing to discuss a variety of approaches including both part and full time roles with flexible hours outside of the traditional GP working day and the opportunity to keep connected to GMS delivery. We want to hear from you whatever your circumstances. The opportunity to shape the development of primary care and competitive pay also makes for an exciting opportunity.

Initially the employing organisation will be Brighton Integrated Care Services (BICS) and the support will be to a Cluster. The GP will form part of the cluster team and the duties of the role can be found in the attached job description. BICS having a managing role in the Benfield Valley Healthcare Hub provides an opportunity for you to continue providing GMS delivery and direct patient care by combining the Proactive Care role with GP sessions in the practice base. Successful candidates will be expected to help shape this role as we learn more about the support our patients need.
