Job Description

CQC - NCH&C’s ‘Good’ rating from the CQC reflects the quality and experience of our staff, their high standards, the delivery of compassionate care to our patients, and timeliness of providing the right care at the right time.

Redeployment -Please note that in the first instance priority for this vacancy will be given to NCH&C staff who are on the Trusts Redeployment Register. Priority over external applicants will be given to other NHS applicants who apply using a restricted account as they are ‘at risk’ or ‘affected by change’.

DBS Update -If this post is subject to a Disclosure and Barring Service (DBS) check and you are not a current employee of Norfolk Community Health and Care NHS Trust then the cost of the DBS check will be automatically removed from your first month’s salary. However if you are registered with the update service this may not be required.

We are seeking a dynamic experienced Community Nurse to join our Coordination Centre team, based at the Priscilla Bacon Centre. You will have experience in triage, assessment, planning and arranging intensive care packages for palliative and end of life patients based on individual patient needs.

The successful candidate will be a registered nurse Level 1, have excellent communication skills and a wide knowledge of health and social care services. He/she will coordinate the Care at Home Team and the referrals to the Marie Curie service.

Fixed term until December 2017 or secondment would be considered.
