Care co-ordinator

Job Description

The Care Coordinator will report to the management team and be responsible for the coordination of the care delivery team, to ensure provision of commissioned services to the people we support. They will be the first point of contact for day to day requirements of the care team as well as the people we support.

Our ideal candidate will have the ability to multitask as they will be working in a busy fast paced office environment, they will need to be able to work unsupervised and must be capable of dealing with People We Support and their families, care team staff and other healthcare professionals. They must also be prepared to participate in our on call service on a rota basis for weekends and evenings.

You will have a good level of education and experience in working in a healthcare or domiciliary care environment in a coordinator or similar role.

An NVQ Level Three is preferred but not essential.
