Salary: £ 26,302- £35,225 pro rata

Hours: 18hrs per week

Location: Edward Jenner Court, Gloucester

An opportunity has arisen for a Band 6 RN to join the Care Home Support Team, acting as Deputy Manager, to lead a team of RNs to deliver a physical health in reach liaison service(PHILS) for patients at Charlton Lane Hospital. The hospital has three wards which provide specialist assessment, treatment and care for older people with dementia and functional mental health problems so the successful candidate must have experience in this area.

The RNs from the Care Home Support Team (CHST),review all new admissions to Charlton Lane and work collaboratively with patients, GPs, ward staff and the Elderly Care Consultant , to develop person centred care plans which aim to maximise health and well-being for those patients who have pre-existing long term conditions.

The team provide clinical support to ward staff and assist them to develop a management plan for those patients who are acutely ill. A key element of the P.H.L.S is the training and education of RMNs and HCAs at Charlton Lane, supporting them to develop knowledge, clinical skills and competencies eg in wound care and palliative care.

We are looking for enthusiastic candidates, dedicated to providing good quality patient care, to join the CHST team. The successful candidate will have knowledge of current issues affecting older people’s physical/mental health and government policy/NICE Guidelines/CQC regulations in the care of older people, both in hospital and Care Homes.

This post offers an opportunity to use your nursing skills in a different environment and to work autonomously without direct on site line management. Excellent communication, the ability to devise and deliver bespoke training sessions and the ability to work well in a team, are essential. There is a commitment to staff training, supervision and appraisal with opportunities for continued professional development.
