Adult Care Services are recruiting to 2 Case coordinator posts which are based in Truro. The posts have different requirements and we are inviting applications for one or both of them.

These are busy and rewarding roles where you will be required to work to a high standard in a demanding environment. Care & Support Older Adults - 37 hours (permanent position)

The post holder will be aware of how to access community based preventative and well-being services i.e. Living Well, Community Directory and community / voluntary sector. You will also be aware of providers of long-term care, both domiciliary and care home settings.

You will need to understand the needs and challenges of people who are older and/or people with disabilities and their carers. You will be required to undertake assessments of need and assist people to plan the support required to meet eligible social care needs.

You will manage your work in order to provide effective assessment, support planning and reviews with regard to cost efficiency. This means delivering services in line with customer, legislative, contractual and strategic objectives.

This is an exciting opportunity for an organised, creative, assertive and resourceful person who is able to manage their own workload without direct supervision.

Working knowledge and experience of the application of the principles and practices of the Mental Capacity Act/DoLS are essential including, knowledge of DoLS authorisation documentation and conditions, familiarity with Court of Protection processes and the ability to effectively describe and communicate aspects of the act to the person, their family and wider care services.

The DoLS Team’s main function is to maintain the human rights of incapacitated adults who are necessarily restricted in residential or hospital care settings. This is achieved by administering a prescribed legal process under the Deprivation of Liberty Safeguards. The team also complete Mental Health Act assessments for older people, people with a Learning Disability and children.

Deprivation of Liberty Safeguards (DoLS) Team - 37 hours (18 months fixed term contract).
