CHC Nurse Assessors

Herts Valleys CCG is looking for dynamic experienced nurses with good communication skills who are interested in developing their leadership and management skills as well as extending their current portfolio of skills and ability to fill the role of Nurse Assessors in our recently transformed Continuing Healthcare service. We are seeking individuals who are registered nurses with the experience of undertaking care needs assessments, case and risk and management in supporting patients and their families through Continuing Healthcare process.

To achieve this you will require working knowledge and experience of the National Framework of Continuing Healthcare and associated tools.

You will be supported in achieving further professional qualifications and development in these key and exciting roles.

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £961 and a maximum payment of £1,665 per annum (pro rata where applicable). Please note this does not apply to VSM (Non Agenda for Change vacancies).
