Children and Young People Continuing Care Nurse Assessor
Full time/part time or job shared considered (if appropriate/suitable interest)
We are looking for enthusiastic motivated nurse to be part of a skilled nursing team within the Children Continuing Care Team within ABMU.
You will be required to utilise your expertise in assessing the holistic needs of children and young people who have been referred for or already receive a bespoke continuing care package in line with the Welsh Government Children and Young People Continuing Care Guidance (2012), encompassing a family centred care approach and in collaboration with th Multi disciplinary Team.
You should be a Registered learning Disabilities Nurse, Registered Sick Children's Nurse or Registered Nurse Child Branch with evidence of previous community experience is desirable , though not essential and commitment to continuing professional development and education.
You will be expected to work across ABMU.
You need to have the ability to travel between sites in a timely manner.
The ability to speak welsh is desirable
