The Children’s Continuing Care Service for Nottingham City is looking for an experienced Paediatric Nurse to join the team as a Case Manager – Band 6.
The Case Manager will ensure that the health care needs of children and families eligible for Children’s Continuing Care are identified and met in accordance with the National Framework for Children and Young People’s Continuing Care 2016.
