We are looking for highly motivated Clinical Lead Nurse to join our community team. Our diverse and friendly team provide a wide range of nursing care to patients in their own homes. We work collaboratively with multidisciplinary teams and social care partners to provide holistic care to patients.
The clinical lead nurse will be a specialist practitioner using their accountable role to drive service improvements and support the educational needs of the workforce. They will interpret and adapt high impact evidence based interventions providing these in peoples own environment.
The Cluster Clinical Lead will lead in:
• Maximising health and wellbeing. Helping people to stay independent
• Working with people to provide positive experiences
• Delivering care and measuring impact
• Building and strengthening leadership
• Ensuring the right staff with the right skills are in the right place
• Supporting positive staff experiences.
An enthusiasm for developing your clinical skills, shared learning and a passion for providing excellent care is key. Previous experience or knowledge of working with a multidisciplinary team is required.
