St Christopher’s delivers empowering and compassionate patient and family care including a rehabilitation model of palliative/end of life care and the use of outcome measurement tools (OACC) to ensure care is effective and needs driven. We work with patients and families on what is important to them.
Most of our care is delivered within the community either in an outpatient clinic or at home, with our beds as a back up to that care. Our services provide expert end of life care 7 days a week.
We have amended our ‘out of hours’ service with the introduction of a community advice and visiting service from 2-10pm. Overnight, our advice and visiting service to the community is delivered from our inpatient unit who visit patients at home.
We actively support and develop all our nursing staff and work in partnership with Kings College London, courses include foundations in palliative care, advanced assessment, independent prescribing and cognitive behaviour therapy (CBT) for the support of people with end of life needs.
Do you want to make a real difference for people at the end of life?
