An exciting opportunity has arisen in Medway Community Healthcare CIC’s Community Palliative Care team. Our palliative care service includes a 15 bedded hospice, hospital palliative care team, day hospice and therapy team as well as the community team where this role is available.
We are looking for a palliative CNS who is passionate about providing excellent palliative care advice and support to patients, MCH colleagues across Medway, community nurses in Swale locality and primary care across Medway and Swale. Whilst you would have a specific patch to enable good relationships and continuity of care to patients to be established and maintained, no day is the same so if you crave variety this role is for you! You would offer advice by telephone in the office as well as regular clinics and making home visits. We are establishing a fully supported programme to ensure all specialist nurses have completed the advance nurse assessment course and are independent prescribers. You would be expected to participate in the on call rota and work weekends on a rota basis.
There is the potential that this is a B6 development role for the right candidate who is already an experienced band 6 wishing to becoming a clinical nurse specialist with the right support, training and experience. We are recruiting to one full time post at 375 hours and one part time post at 225 hours.
