Clinical Nurse Specialist/Specialist Discharge Planning

An exciting key role has arisen, working across the Royal United Hospitals Bath NHS Foundation Trust (RUH) and with NHS Bath and North East Somerset Clinical Commissioning Group (BaNES CCG) to ensure a smooth and effective inpatient discharge process. The post holder will support delivery and development of processes to ensure discharge services are of high quality and value for money. Key to the role is developing and maintaining relationships with other managers, healthcare professionals, the local authority and voluntary sector agencies.

The post holder is identified as a clinical nurse specialist or specialist allied health or social care professional and will work directly with the Integrated Discharge Service Manager at the RUH and with the Deputy Director of Nursing and Quality at NHS BaNES Clinical Commissioning Group (CCG).

The post holder will be required to case manage a cohort of patients registered with BaNES CCG GP practices requiring coordination of discharge planning through Continuing Health Care Fast Track and/or self-funding packages of care as well as supporting the elimination of delays for other in-patients in order to safely reduce excess bed days
