An exciting opportunity has risen for a dynamic, enthusiastic Health Care Assistant interested in working in the community within the Severnvale / Yate Locality.

The District Nurse team is a community nursing team providing a flexible nursing service to patients in their own homes, 7 days a week, including bank holidays between the hours of 0800 and 2000.

The Community Health Care assistant will support the district nursing team to provide assessment, development, implementation and evaluation of programmes of nursing care in the community through providing quality and good standards of care to service users. The team is friendly, supportive and hardworking and have a real commitment to delivering excellent patient centred care as part of the wider multidisciplinary team.

We are looking for HCA with good decision making, prioritisation and time management skills who can work alone but with the support of a nursing team to deliver compassionate, dignified care. You will also need excellent communication skills including negotiating, diplomacy and an ability to deal with sensitive issues.

You will work closely with the nursing team and need to have a good clinical skills (included supporting palliative care, wound care, safeguarding) and you will need to be able to form good working relationships with all levels of staff including GP practices and hospital services.
