Job Description

Job title: Community Lead Nurse for Complex Care (Band 6 equivalent)

Reporting to: Business Centre Manager

Location: Community based between Bath

Hours of work:  27.5 – 37.5 per week

Salary: £28-32k, OTE £34k (pro rata for part time)

If you are a qualified and experienced nurse with at least 2 years' clinical working experience, gained through working in the NHS, Nursing Homes, Community or District Nursing, or through working in Homecare Environments (Complex and Non-complex care) then this is the perfect role for you.

This is an exciting opportunity to grow and share your excellent nursing experience and knowledge.  You’ll be helping people to live independently in their homes and achieve their goals through care planning and assessing their needs, alongside supporting a dedicated care team to ensure quality care is delivered.

Personal and professional development is actively encouraged and the role is diverse, covering a wide range of complex conditions for an equally wide range of clients and ages.  Upon completion of induction and training you will manage and assess all referrals, ensuring all assessments, care plans and associated documentation is in place and regularly updated.
