You will be providing case-management, care co-ordination and support to patients in their own homes.

The main responsibilities of the post:
• The post holder will work as a case manager, without direct supervision.
• They will be involved in delivering integrated care project working with GPs, Social Services, mental health service and the Community Nursing Service.
• They will be providing case-management, care co-ordination and support to patients in their own homes.
• They will attend integrated care meetings with the integrated team members to discuss and co-ordinate care.
• They will be responsible in ensuring the service meets it referral targets and maintain systems such as health analytics’, RIO and completing monthly reports to the service lead.
• They will work closely with and oversee day to day management of the Integrated Care Liaison Officer.
