Job Title: Mental Health Nurse Band 5

Salary band: £ 21,909-28,462 per annum

Hours: 225 hours per week vacancy available

Location: Waterford House, New Milton Hampshire.

Duration: permanent

Community mental health care is changing, and we are looking for a dynamic, skilled and motivated nurse practitioner to join us and play a vital role in delivering seamless care for our communities. Working as part of the Community Mental Health Team we have the following vacancy;

1 part time Band 5 Nurse Main duties- Care Coordinator

This West area has many attractive facilities and amenities, being close to Southampton and Winchester city. We enjoy easy access to the Solent coast with ferry to the Isle of Wight and border on the New Forest national park. Good links to London by road (M3) and rail (direct to Waterloo) as well as international airports at Southampton and Bournemouth.

Key aspects of the role include:

  • Leading and contributing to the process of service user assessment as part of access to secondary care.
  • Undertaking brief intervention in line with the care plan, where longer term intervention is not indicated
  • Delivering higher level intensity community interventions as part of the integrated Shared Care model
  • Formulating care plans in line with assessed need
  • Working in a manner which is helpful to the service user and the referrer, based on principles of recovery, personalisation and choice.
  • Working with individuals in the least restrictive manner possible.
  • Deputising for the Team Leader as required.
  • Working in a service which may include some evening, weekends and bank holidays.

You will have previous experience in the effective and positive management of people with mental health needs and will be able to evidence effective team working and leadership skills.

Ability to drive and use of a car for work purpose is essential in this role.
