Community Nurse

Job Description

There has never been a more exciting time to join one of the largest specialist community NHS providers in the country – one that is also among the very best NHS employers as voted by its own staff.

Over the next few years the way the NHS works will change; care out of traditional hospital settings is quickly becoming a much larger part of what the NHS does. This change is happening right across the country and as a leading community provider our aim is to be at the forefront of this exciting future, joining-up services in local communities to better meet the individual needs of our patients.

DCHS is a top performing NHS Trust and we have Community Staff Nurse roles available at Amber Valley, South Derbyshire, Erewash, High Peak & Dales, NE Derbyshire, Chesterfield and Derby City. We also have opportunities for you to rotate between sites within the locality.

The successful candidates will have high levels of personal and clinical credibility and the ability to instantly form effective working relationships in a range of work settings. You will have the opportunity to really make a difference in ensuring that quality services are delivered both now and into the future based around our patient needs. We will also work with you regarding your ongoing development and revalidation.

Can you demonstrate good communication skills, have the ability to plan and organise competent, evidence based clinical care delivery? If so, come and join us. We look forward to receiving your application form.

As a clinical employee it will be your responsibility to ensure that your behaviour and values reflect those set out in the DCHS Way outlined on our website and in the additional information provided.

Those patients, their loved ones and our many colleagues must be at the heart of our ‘future NHS’. Local commissioners of health and care feel the same, so together we’re doing all we can to understand public, patient and professional opinion on a range of exciting proposals to change the way the NHS works in north Derbyshire.

If this consultation outcome does affect this position, successful applicants will be guaranteed a position with the Trust following the outcome of the public consultation albeit perhaps in another nearby location. However, we are not expecting any changes to take effect before April 2017.

Derbyshire Community Health Services NHS Foundation Trust (DCHS) is one of the largest providers of specialist community healthcare in England and we currently employ around 4,500 staff. We deliver a variety of services across Derbyshire, caring for nearly 18 million patients each year.

We deliver patient care through 11 community hospitals, 4 minor injury units, 29 health centres and a learning disability centre. Another key part of our service is out in the community where we care for people in their own homes via therapists, health visitors and district nurses, and promoting public health in the wider community. As a Trust we care for patients of all ages with a special focus on the needs of elderly people and the very young.

DCHS has been recognised as a top 10 NHS organisation to work for in the UK by the Health Service Journal and NHS Employers in their Best Places to Work 2015 campaign. The Trust is also the runner-up in the category for the best NHS community trust employer.
