We are looking to recruit a number of Senior Community Rehab assistants to join our and support our Rapid Response team at Camberley Health Centre, covering the Surrey Heath locality. We are happy to consider full and also part time working within the service.
You will work within our teams based either at Milford Hospital (Guildford area) or Camberley Health Centre(Camberley area) . The Integrated team comprises of: Therapists, Nurses, Community Rehabilitation Assistants, Social Care, Mental health and Community Nursing colleagues plus Voluntary sector staff.
Rapid Response provides assessment and short term rehabilitation to patients in their own homes. As a service our focus is around admission avoidance, and the coordination and delivery of packages of care to support short-term goal-orientated rehabilitation. Rapid Response also deliver a falls service.
You will have the opportunity to develop your expertise working within a community setting with patients with a range of conditions plus work alongside Therapists and Nurses within the team with a focus on working to specific patient centred care plans. This will enable you to further develop your rehab skills and work towards caseload working.
The role will also involve other duties such as overseeing daily diaries and emergency plans, plus assisting with the coordination of staff rotas and lists for visits. As a senior there will also be the opportunity to provide a supportive role to Community Rehab assistants working within Rapid response.
All applicants require a willingness to undertake NVQ level 3 in Health and Social Care. They should be able to demonstrate good personal/interpersonal skills and demonstrate their ability to work effectively in an integrated team.
They should have evidence of knowledge and practical experience in an acute or community setting and should be able to demonstrate the ability to manage/coordinate their own caseload work within an integrated model delivering community services.
The successful applicant would require to a good knowledge of a wide range of clinical conditions with a focus on work within community and rehabilitation settings.
The need to be a car diver and have access to a vehicle is an essential requirement of the position.
