An exciting opportunity for a Physiotherapist to join a new multi-disciplinary team, delivering North Lincolnshire CCG’s community respiratory service. Full- or part-time opportunities available.
Working from community venues across the CCG area, this role will lead in the delivery of pulmonary rehabilitation programmes, supported by Technical Instructors and Nurses from within the MDT.
The programme involves conducting patient assessments, delivering exercise and education to patients within a group setting, and providing self-management support in clinic or home settings, or remotely by telephone or using innovative technology developments.
The pulmonary rehabilitation programme is just a part of the wider community respiratory service, which includes Home Oxygen Assessment & Review and chronic respiratory disease management. As a result, we can offer are multi-disciplinary development opportunities across the other elements of the respiratory service.
This role will work with the service’s Team Leader and Respiratory Nurse Specialist, with input from the MDT, to ensure improved management and outcomes for people with respiratory disease in North Lincolnshire. The role holder will be required to manage their own caseload and promote the service locally to increase appropriate referrals.
The position reports to the Team Leader. A team office is located in Scunthorpe.
