Community Staff Nurse

Job Description

We are seeking a willing, enthusiastic and highly motivated registered staff nurse to join our community nursing service. The successful applicant will be committed to working closely with health and social care colleagues to provide high quality care to patients with a wide range of medical conditions in the community setting.

This role requires excellent communication skills as you will be undertaking complex holistic nursing assessments and developing and delivering programmes of care for patients and carers.

You will work collaboratively with other professionals, voluntary sector and informal carers to promote excellent care to patients in their own homes and community settings.

You will be joining an enthusiastic and motivated team who are friendly, approachable and supportive. We are a highly skilled team delivering a wide variety of clinical skills to meet the increasing dependency and acuity of patients now nursed in the community. We are very patient focussed and staff development is high on our agenda. As part of our team we want to develop all staff to the highest standard; you will receive regular one to one supervision meetings and an annual appraisal to support and monitor the development of valuable skills.

You will be required to work flexibly to cover the health needs of the local population including weekends, bank holidays, evenings and nights.

The Trust is committed to safeguarding adults and children therefore if the post is subject to the Rehabilitation of Offenders Act (ROA) 1974 (Exceptions) Order 1975 (as amended) the successful candidate(s) will be subject to a criminal records check with the Disclosure and Barring Service (DBS). The cost of the DBS must be met by the successful candidate(s) through salary deduction. DBS charges are as follows if applicable to the post:

Standard Check: £2600 + Administration cost of £400 = deduction from salary as a one off payment or over 3months at £10 a month

Enhanced Check: £4400 + administration cost of £400 = deduction from salary as one off payment or £15 deduction from 1st months salary, £1100 deduction from salary over following 3 months.
