Come and work in a bespoke, innovative older peoples integrated care system project that King’s College Hospital Foundation Trust, (Princess Royal University Hospital site) and local community providers are launching in January 2017. Be at the beginning of this innovative, integrated service design for older people within the London Borough of Bromley, where you can influence strategy and share a passion for patient centred, fully integrated Gerontology care. Take up this unique opportunity of being part of this forward thinking development for frail patients.
A unique, newly refurbished bed and chair provision at the Orpington Hospital site for frail older people will open in January 2017. Three GP-led Integrated Care Networks will forge strong and effective care links with the acute hospital for the frail older patient. There will be many opportunities to develop clinical pathways, GP liaison systems and MDT working in the community to provide excellent patient centred care and experience to the frail local population, including those in Care Homes.
