An exciting opportunity has arisen for an experienced nurse to join the Nottingham Citycare Partnership Continuing Care Team as a Continuing Care nurse Co-Ordinator. Nottingham CityCare Partnership, is the leading provider of NHS community services in Nottingham; a staff-led social enterprise; trusted to connect with and care for people, to improve the health and wellbeing of the community.
Nottingham CityCare is looking to recruit a forward-thinking, resourceful, experienced and confident nurse to the post of Continuing Care Nurse Co-ordinator. The role of the nurse Coordinator will be to facilitate the implementation of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care within Nottingham City.
The role will involve providing advice, undertaking health needs assessments and coordinating continuing healthcare funding. You will carry out work autonomously and in partnership with health and social care professionals in arranging care packages and placements. You will be part of the Continuing Care Team which is based at Newbrook House. The Co-Ordinator will primarily cover the Nottingham hospitals and will also work in the community.
