Job Description

'Keep Well' programme pathways – vulnerable populations in non GP practice settings

This short term six month part time (15 hours per week) post will support partner organisations to embed Keep Well in their own processes. If you would like the opportunity to build your experience of developing pathways and working with partner agencies and organisations to review, evaluate and further develop practice that will address health inequalities in practical ways for clients and patients.

This will involve being able to identify and offer solutions to barriers to adopting a client centred/holistic approach within services across public and voluntary sectors, strengthen existing and develop new pathways for enabling services to have more clarity of who, how and what can help their clients/patients. A key element will be to enable plans for evaluation of these pathways.

You will have a relevant degree, e.g. health sciences, allied health professions, nursing, social sciences or community development, relevant experience, excellent communication and interpersonal skills, as well as being able to prioritise your own workload.
