Following a scheme for investment in Primary Care services, we are delighted to extend our community based diabetes service provision.

The service requires enthusiastic and motivated applicants for a Diabetes Specialist Nurse to work along side existing Primary, Community, Emergency & Third Sector services.

The Diabetes Healthcare Technician, will embark on a series of pre defined initiatives to improve the skill and knowledge about diabetes management and self care for both health professionals and people living with diabetes in the North Denbighshire Area

The service is to improve provision for preventing and managing diabetes within primary care, to prevent avoidable admissions to hospital and reduce referrals into the traditional diabetes outpatient services. The Locality based diabetes service will significantly contribute to both improving outcomes for patients through the delivery of approved Structured Diabetes Education programmes (principally for Type 2 diabetes) and containing spends related to diabetes in the coming years.

With effect from 1st January 2016 points 1,2,3 and 4 on AfC Pay Band 1 and Pay Band 2 will be adjusted to £16,132 to reflect the incorporation of the living wage.
