We are seeking to appoint an enthusiastic District Nurse to join Community Integrated teams across Sunderland.
Sunderland is part of the Vanguard All Together Better project and our integrated health and social care teams are committed to implementing new models of care delivery across the Sunderland communities.
We aim to work across traditional boundaries to provide integrated care in order to deliver a standard of care we would want for ourselves, our families and friends.
This post presents an ideal opportunity to play a role in our future service delivery of District Nursing within Sunderland; the aim of the service is to ensure that people receive the right care, at the right time by the right person.
We are therefore seeking an individual with a positive and proactive approach to lead and develop staff within the Integrated teams ensuring the delivery of patient centred care.
The individual will be required to possess up to date clinical skills and demonstrate effective communication across a wide range of disciplines.
Applicants must be a registered nurse and possess the relevant qualifications required; Cert/Diploma/Degree in District Nursing or equivalent.
The postholder will be expected to assess the health and health related needs of patients, clients, their families and other carers and identify and initiate appropriate steps for effective care for the individual and groups.

Individual salary will be determined within the Band range based on previous skills, knowledge and experience.
