District Nurse – Caseload holder Fulltime (375 hrs) and Part-time hours available. Job shares considered.

We are pleased to offer this opportunity to come and work as a band 6 District Nurse within Stockport Community Services

We’re looking for experienced, passionate and highly skilled District Nurses to shape, form and energize this innovative, patient led service. The post holder will be part of an integrated team responsible for the planning, implementation and evaluation of nursing care needs and the delivery of high quality care. The post holder should have a broad range of clinical skills, good organisational and assessment skills, be innovative, self-motivated, flexible and willing to cover the service which is provided 7 days per week. You must hold the Specialist Practitioner Qualification in District Nursing and be a nurse prescriber. You will be supported in your role by an integrated team manager.

Stockport NHS Foundation Trust (Community Nursing) has a reputation for providing quality care and a willingness to invest in the education of its staff. We believe in the development and support of our staff by providing preceptorship, mentorship and coaching. We also have a range of educational opportunities and candidates will be supported in undertaking further study.

In the coming year we will be a Multi Specialty Community Provider organisation which brings together Stockport GPs, AHPs, other health workers, social care staff and voluntary partners who will work together, sharing information and taking a more coordinated approach to caring for people living in the local community. Our integrated nursing teams will also contain social workers and specialist nurses. Stockport Foundation NHS Trust hosts one of four ‘specialist’ hospitals in Greater Manchester. Being a specialist hospital will enhance our general surgery, anesthetics, critical care and emergency medicine for the benefit of people in Stockport, High Peak, Cheshire and across Greater Manchester but will be a centre of excellence for elderly care and cancer.

We will work closely with our hospital to ensure each patient has a good experience of care and is helped to return and remain at home after it.

We would welcome applications from students who are due to qualify as a district nurse specialist practitioner.
