The post holder will assist in the implementation of the National Framework for Continuing Health care (2014) and manage the process for Continuing Health Care within AMH & LD CPG. They will ensure that appropriate monitoring review procedures are implemented and maintained as set out in Government guidelines.The post holder will play a supporting role in providing clinical advice in all aspects of the Continuing NHS Care process to a locality team.

The post holder is required to act autonomously throughout the assessment process to ensure that healthcare funding policies are adhered to in accordance with the Welsh Assembly Government Framework, using analytical and judgemental skills to interpret these for each individual case.

The post holder will develop a proactive role within the community and Independent sector; acting as liaison between providers, Primary care & Secondary care Services, monitoring and enhancing quality of care delivery.

The post holder will, on behalf of the Locality Team scrutinise individual packages of care for CHC/ and Joint Funded individuals, balancing financial constraints and the assessed care needs of patients; use of NHS resource and maintaining value for money.

The ability to speak Welsh is desirable for this post.

With effect from 1st January 2016 points 1,2,3 and 4 on AfC Pay Band 1 and Pay Band 2 will be adjusted to £16,132 to reflect the incorporation of the living wage.
