An exciting opportunity has arisen for a Band 5 Health and Social Care Co-ordinator to join the Integrated Hospital Discharge Team based at Derriford Hospital.

The key part of our service is providing high quality assessment and onward care planning as part of an integrated team, to meet the needs of the person to leave hospital. You will be responsible for triaging new referrals for our single point of access for Health and Social care referrals into the Integrated Hospital Discharge Team. You will also be able to manage a caseload of patients that will require social assessments to enable them to have a safe discharge home.

You will need the ability to work autonomously and as part of a team demonstrating initiative with sound clinical knowledge and judgement.
