Job Description

Cheshire and Wirral Partnership NHS Trust (CWP) provides specialist child and adolescent, adult and older person's mental health, learning disability and drug and alcohol services to a population of approximately one million people. CWP also offers community physical health services to patients who have a GP in Western Cheshire.

We are seeking to appoint two enthusiastic, motivated, reliable Community Health Care Assistants and also Bank Staff who are keen to support and deliver a range of quality nursing services to our patients within the District Nursing Out of Hours Service which is based on the Countess of Chester Healthpark and visits patients, and their carers, of all 9 Integrated Community Care Teams across Western Cheshire
Working within our integrated teams, you will undertake duties involving the delivery of nursing services using a person centered approach. This includes providing quality care to housebound patients or patient's in long term settings, avoiding unnecessary admission to hospital or long term care.

An enthusiasm to support our teams to deliver high quality services is a must for this role and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable and approachable.

The hours will be worked as 7 shifts per fortnight from  ( Phone number removed ) . Their may be the opportunity to work night shifts as part of the role. Bank staff are also required to work evening or night shifts as required

Disclosure and Barring Service Check: Successful candidates who require a DBS check will be required to pay for the cost of the DBS Disclosure which is £44 for an enhanced disclosure and £26 for a standard disclosure, the cost of which will be deducted from salary immediately following appointment.

DBS Update Service: If the role you are successful for requires a DBS check, you are encouraged to enrol for this service. A small annual fee of £13 per year applies. Please see attached “Update Service Applicant Guide” document for further information
