The Active Ageing service is a new and developing Team that has been operational in Bath and North East Somerset since March 2014 and since January 2015 in South Gloucestershire. Active Ageing team is part of Sirona’s care and health’s Adult services, and works within the Sirona Model.

Our work is underpinned by the four principles of health visiting and is a public health preventative role, focusing on improving health and reducing inequalities, with attention to social and emotional, as well as physical health We aim to offer high quality, person centred care to everyone we visit between the ages of 80-84 years old. By working in partnership with our clients, we aim to support healthy ageing and actively promote the health of older people supporting them to be independent, to enjoy life, to live well with complex co-morbidities, dementia and frailty and to ensure their voice is heard.

Our vision is to be involved in the cultural changes to services and systems supporting older adults. So that they are valued as an asset, their voices are heard and they are enabled to enjoy full and positive lives in their own homes.

We are seeking to recruit a Health Visitor/ Specialist Public Health Nurse for 225 hours over 3 days. Although the post will be based at the Patchway Clinic, if the need arises the post holder will cover other areas in South Glos.
