An exciting opportunity has arisen for dynamic nurses to become part of our new Hospital at Home service. You will be part of a virtual ward which provides facilitated early discharge for patients who are in hospital and able to continue their recovery in their own home as well as preventing admission to hospital where possible. Patients are offered continuity of care by following them from hospital back home and these posts provide the opportunity to work more closely with community and primary care colleagues to provide a seamless service.

You will have the ability to work independently with indirect supervision and as part of a very supportive multidisciplinary team to care for patients in their own homes, ensuring a person focused, individualised approach for patients with complex, sub-acute, medical, functional and social needs. You will also be able to prioritise and work flexibly to meet the demands of the service.
