Are you an experienced healthcare professional who has community/primary care experience?
Taurus Healthcare are looking for motivated, flexible and committed individuals to join the successful initiative of Integrated Care Practitioners in Herefordshire.
The service bridges between primary, community and secondary care, focusing on sharing information, care planning and supporting patients and their carers to prevent unnecessary admissions.
The roles are linked to GP practices across Herefordshire and utilise exciting ways of working through mobile patient records.
You will have community/primary care experience holding a registration with either the Nursing and Midwifery Council or Health Care Professional Council. There is an expectation for all ICPs to have key core clinical competences, but the underpinning values of care are applicable to all disciplines.
The successful candidate will be experienced in caring in an autonomous manner and have the highest values and standards in care. Non-medical prescribing and health assessment qualifications would be an asset, but not essential.
