The Primary Care and Community Services Delivery Unit, Abertawe Bro Morgannwg University Health Board together with Swansea Social Services are seeking to appoint an Integrated Community Services Manager, to operationally and strategically lead integrated teams of District Nurses, Social Workers, Therapists and Domiciliary Care Services, as well as taking strategic responsibility for the development of new health and social care services where indicated necessary.

The community cluster networks are rapidly becoming the focus of support in the community and the teams within the clusters predominantly (but not exclusively) work with older people and the issues of frailty, and with physically disabled people who are affected by long-term complex and adverse conditions.

The Integrated Services in Swansea have a matrix model of professional leadership and as a result of this structure this post is restricted to a Health professional.

The Integrated Community Services Manager’s (ICS) main function is to operationally and strategically lead the delivery of a range of preventative, pre-emptive, reactive and rehabilitative community health and social care services, as well as the arrangement and delivery of appropriate long term care and support to the residents of the community cluster networks. The post will also have prime responsibility to lead on the implementation of the optimal Western Bay Model for Intermediate Care, taking lead responsibility for the pooled fund and associated performance reporting functions.
