Due to the integration and expansion of the service, an exciting opportunity has arisen within the newly established Integrated Discharge Service (Complex Case Management).

The IDS Therapy work stream is a new development to support the discharge to assess model of care. Working between hospital and community locations to discharge patients home at the earliest beneficial opportunity.

The successful applicants will support organisational improvements in discharge planning and patient flow so that East Lancashire Hospitals Trust can deliver its objectives of Safe, Personal and Effective care delivery through sage, appropriate and timely discharge.

We are looking for a highly motivated individual with outstanding leadership skills, excellent communication skills and the ability to operate and manage within a team to provide an organisational wide service. The post holder must have a detailed understanding of discharge planning processes as well as recent experience within an acute hospital environment.

The post holder must have the confidence and skills to work effectively across organisational boundaries and understand the complex priorities that this role entails.

It is important you are made aware of the changes which East Lancashire Hospitals NHS Trust has implemented with regard to charging for the cost of undertaking Disclosure and Barring Scheme (DBS) Checks (previously known as Criminal Records Bureau or CRB) and how this may impact upon you.

As this position is exempted from the Rehabilitation of Offenders Act 1974 and involves work that is identified as a regulated activity as defined in the Safeguarding Vulnerable Groups Act (2006), any offer of employment is provisional upon a satisfactory DBS check being obtained.
The cost of an Enhanced Disclosure will be £4800, which includes a small handling fee.

In line with other NHS organisations in the North West Region, the Trust is now passing the charge for undertaking a DBS check on to candidates in the event they are successfully appointed into the post for which they have applied. Candidates can choose whether to pay this over 1-3 months as a deduction from the monthly salary.
