Complex Case Practitioner

We currently have vacancies for Integrated Team Complex Case Practitioners to join forward thinking Integrated Health and Social Community Teams working for Central Cheshire Integrated Care Partnership based in the South & Vale Royal areas. The post is offered at either Band 6 as a developmental post or at Band 7 for an experienced nurse with the appropriate qualifications and experience.

Post 1 – 375 hours. Grosvenor & Hungerford Road. Crewe.

Post 2 – 375 hours. Worked across Firdale and Danebridge Medical Centres, Northwich. Job share considered.

Post 3 - 375 hours . Alsager.

The successful candidate will work closely with the Community Matrons and in partnership with primary care to provide advanced clinical assessment and clinical case management for patients with complex long term condition care needs living in the community, who are at risk of deteriorating health that may result in declining quality of life or unnecessary hospital admissions.

It will be expected that you will have strong leadership, communication and decision making skills and an understanding and commitment to the modernisation of community nursing.

Whilst specific bases are identified all staff will be expected to work across the organisation as service need dictates.
