An exciting opportunity has arisen for an experienced nurse, RGN, to join a dynamic team of nurses that provide commissioning services for people with complex care needs who are eligible for NHS Continuing Healthcare funding or NHS-funded nursing care. The candidate should have sound knowledge of the NHS Framework for Continuing Healthcare.
The successful candidate will support the assessment process for NHS Continuing Healthcare / NHS-funded nursing care in line with the NHS Framework for Continuing Healthcare. A key aspect of this post involves line management and supervision of the locality CHC team, working closely with local partners including health, social care and independent providers.
Experience of undertaking assessments, and working with patients, families and carers is essential. The CHC team are responsible for the assessment and review of people who reside in nursing care homes and the clinical commissioning of packages of care for people with continuing healthcare needs who reside in their own homes.
As Team Leader in the CHC Team the post holder will support the Team in driving key areas of service delivery through the implementation of effective processes, quality decision making, and compliance with legislative frameworks.
