Join our Mental Health Team at HMP Warren Hill/HMP YOI Hollesley Bay as a Mental Health Nurse and help us positively change the way healthcare is viewed within the prison estate.
Based in a secure setting, you will have the opportunity to deliver individualised, evidence-based, patient centred nursing care to a unique and varied population.
Care UK is the Health in Justice market leader, providing healthcare services to offenders across the country. Our model of care is our Wellbeing Approach – an integrated, comprehensive approach in recognition of the fact prisoners are a socially-excluded group with healthcare needs which are complex and often associated with poor outcomes. It adopts the premise no single health/wellbeing factor can be taken in isolation of other motivational issues which might impact a person’s care.
We’ll utilise the wellbeing model to put patient care and safety at the heart of everything we do through assessing prisoners across 5 domains of; physical and mental wellbeing, substance misuse, relationships and resettlement. The patient and clinician complete a wellbeing wheel assessment to set goals and outcomes in partnership. The patient will complete the assessment and any needs identified will be met by our fully integrated service delivery team.
You will participate in the delivery of holistic care whilst working closely with a multi-disciplinary/professional team. This will involve opening/maintaining/closing and taking part in reviews of individuals being managed under the Prison Suicide and Self Harm risk management system (ACCT).
Responsibility will also be with you to assess physical and mental health of prisoners on admission to the prison so that any appropriate referral can be made.
To be successful in the this role you will have:
•Excellent interpersonal, communication and team working skills
•Experience of key-working and care planning
•An understanding of clinical governance and its relevance to delivering Health Care in a Prison setting
