Park Medical Practice, Lytham St Annes are looking to recruit a reliable and highly motivated Nurse Practitioner to join our friendly team. Hours of working are negotiable, full/part time. You must be a team player, work independently and be flexible, able to work under pressure, must have good communication skills.

We are a busy 3 partner training practice with 1 salaried GP, 2 Nurse Prescribers, 1 HCA, Phlebotomist and a team of supportive and dedicated reception/admin staff. As a practice we value our staff and are proud of how well we work together as a team. We are keen to support career development and training. As a training practice we have ST3s and FY2 students. Our list size is 6300 and growing. We are actively involved in our local neighbourhood and CCG. High QOF achievers. Emis Web clinical system. New purpose build premises. Rated overall 'Good' by CQC.

Essential skills: RGN with current NMC registration, Nurse Practitioner qualification, Non-medical prescribing qualification, Experience in chronic disease management.

Role includes: telephone triage, home visits, referral to community and secondary care services, chronic disease management.
