Band 7 Nurse/Team Lead - Islington Intermediate Care Team
An opportunity has arisen for an experienced and enthusiastic nurse to join the Islington Intermediate Care Bed Based Team. The team includes a physiotherapist, OTs, a rehab assistant and social workers and they are responsible for the intermediate care bed provision and support for Islington residents placed within the borough.
The post holder will be required to lead the multi-disciplinary team providing rehabilitation to patients in the community intermediate care beds at Mildmay and St Anne’s nursing home in a timely manner to avoid delayed transfers of care. The post is based at Vorley Road N19 but may require working across other sites depending on the needs of the service.
You must have a good knowledge of intermediate care provision, as well as experience of managing and supervising staff. You must be an excellent communicator, well versed in multi-disciplinary working and able to develop and maintain relationships with key partners .
Clinical support will be provided by the lead nurse in the Integrated Community Ageing Team and management support from the service lead for Islington community Rehabilitation services.
